Insurance & Payment
My services may be covered in full or in part by your health insurance or employee benefit plan. I will verify your eligibility and benefits when you provide your insurance information to me. Although I make every effort to provide you with accurate information, I cannot be responsible for inaccuracies in the information the insurance provides me. I therefore recommend that in addition to the information I obtain for you, I recommend that you also check your coverage carefully by asking the following questions:
- Do I have mental health benefits?
- What is my deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- How much does my plan cover for an out-of-network provider?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
In most instances, I submit billing directly to your insurance company electronically, This is a very efficient process. Some companies require that I send paper claim forms via US Mail. Still others require that you pay for the services up front and then submit bills to them for reimbursement. I will follow our company's requirement and provide required documentation for you.

Reduced fee treatment is available on a limited basis.
Cash, check, Visa, MasterCard and Discover are accepted for payment.
Some people pay co-pays at the time of service. For everyone, I will send a statement at the beginning of each month. Payment is due by the 15th of the month in which you receive the statement.
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
